Hazard:  Fall from scaffold

Standard:  1926.451(f)(6): This scaffold was within 10 feet of the overhead powerline with 300 volts to 50KV.

Corrective Action:  The scaffold should not be erected, dismantlled, used or altered near energized powerlines.

Hazard:  Fall from scaffold and steep roof. Struck by falling materials.
Standard violated:  1926.451(g), 1926.451(b), 1926.100(a), 1926.451(h), 1926.501(b)(11):
Fall protection not provided. Scaffold not fully planked. Hard hats and falling object protection not provided.
Corrective Action:  Install guardrails on scaffold. Fully plank scaffold. Provide hard hats and falling object protection and steep roof fall protection.

Hazard:  Electrical shock from contact with energized lines.  Fall from scaffold.
Standard violated:  1926.416(a), 1926.451(g): Employee could contact electric power circuit in the course of work and/or fall from scaffold.
Corrective Action:  De-energize, relocate or protect lines.  Use guardrail systems, safety net systems, or personal fall arrest systems.

Corrective Action:  Provide guardrail system, safety net system, or personal fall arrest system.

Hazard:  Employees working on 4-buck scaffold system without fall protection.
Standard violated:  1926.451(g):
Four employees pumping concrete into blocks on parapet. No one wearing fall protection; no rails erected.
Corrective Action: 
Employer stopped job and had competent person erect proper railings.

Hazard:  Fall from scaffold.
Standard violated:  1926.451(b), 1926.451(g): Platforms not fully planked. Fall protection not provided. Planks extend too far past supports.
Corrective Action:  Fully plank platforms. Provide guardrail system, safety net system or personal fall arrest system.  Use planks of proper length.

Hazard:  Fall from elevation.
Standard violated:  1926.451(g)(1): Fall protection not provided for employee standing on guardrail of scissors-lift.
Corrective Action:  Provide scaffold that does not require employee to stand on guardrails or provide a personal fall arrest system. Train employee to recognize fall hazard.

Hazard:  Lack of Fall Protection; Scaffold not fully planked; Lack of Head protection.
Standard violated: 
1926.451(g)(1); 1926.451(b)(5)(i) 1926.100(a):  Apartment complex under construction with unsafe scaffolding.
Corrective Action: 
Install proper planking, guardrails and require use of hardhats.
