Fall Hazards from Scaffolding - Page 2

Hazard:  Fall from scaffold

Standard:  1926.451(c)(2):  Inadequate scaffold foundation.

Corrective Action:  Scaffold poles, legs, posts, frames, and uprights were not placed on base plates and mud sills or other adequate firm foundations.

Hazard:  Fall from scaffold

Standard:  1926.451(c)(2):  Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or adequate firm foundation.

Corrective Action:  Properly install base plates and mud sills on scaffolding.

Hazard:  Hazard of scaffold collapse due to concrete blocks used on footing.

Standard:  1926.451(c)(2)(iii):  Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or adequate firm foundation.

Corrective Action:  Remove concrete blocks, and properly install base plates and mud sills on scaffolding.

Hazard:  Unstable scaffold leg support. No baseplate

Standard:  1926.451(c)(2)(iii):  Concrete blocks used as scaffold base and missing baseplates.

Corrective Action:  Install adjustable screw legs and level ground.

Hazard:  Fall from scaffold

Standard:  1926.451(c)(2)(iv), 1926.451(g)(1):  Employees erected a wooden working platform on two aerials lifts over 30 feet from the ground.

Corrective Action:  Working platform was dismantled and the high lift were used to complete the job

Hazard:  Fall from platform.
Standard violated:  1926.451(c)(2)(v): Secured platform not provided.
1926.451(g)(1): Fall protection not provided.
Corrective Action:  Secure platform to forks and provide fall protection.   Use ladders or supported scaffolds instead of forklift.

Hazard:  Fall from platform.
Standard violated:  1926.451(c)(2)(v):
Employees working from the forks of a rough terrain forklift without protection against falls. Other fall hazards from the building floor.
Corrective Action: 
Provide adequate equipment to perform the job such as a secured and safe platform attached to the forks with approval from the manufacturer.

Hazard:  Fall from platform.
Standard violated:  1926.451(c)(2)(v): Unsecured platform.
1926.451(g)(1): Fall protection not provided.
Corrective Action:  Secure platform to forks and provide fall protection.  Use ladders or supported scaffolds instead of forklift.

Hazard:  Fall hazard.
Standard violated:  1926.451(d)(3)(iX):  2 Point Suspension Scaffold -anchor points not evaluated.
Corrective Action:  Engineering firm hired to design, install and test anchor points for load capacity required.
Hazard:  Fall hazard.
Standard violated:  1926.451(d)(3)(X):  2 point suspension scaffold tiebacks not at 90 degrees.
Corrective Action:  Tie-backs corrected to 90 degrees.

Hazard:  Fall from scaffold.
Standard violated:  1926.451(e): Safe scaffold access for all employees not provided.
Corrective Action:  Provide safe scaffold access such as ladders.

Hazard:  Fall from scaffold

Standard:  1926.451(f)(6):  This scaffold was within 10 feet of the overhead powerline with 300 volts to 50KV.

Corrective Action:  The scaffold should not be erected, dismantlled, used or altered near energized powerlines.
