Fall Hazards from Scaffolding - Page 1

Hazard:  Fall from scaffold; collapsing scaffold.
Standard violated:  1926.451:  Employees working on improperly constructed scaffolding system.
Corrective Action:  Erect proper scaffold system.
Hazard:  Unstable object. (Hay) Lack of fall protection.
Standard violated:  1926.451:  scaffold made of hay.
Corrective Action: 
Use correct material in constructing scaffold.
Hazard:  Fall from scaffold; collapsing scaffold.
Standard violated:  1926.451:  Employees working on inadequate scaffolding.
Corrective Action:  Install appropriate scaffolding system

Hazard:  Fall from scaffold; collapsing scaffold.
Standard violated:  1926.451(a)(1): Scaffold not capable of supporting 4 times intended load;
1926.451(a)(6): Scaffold not designed by a qualified person.
1926.451(f)(3): Scaffolds not inspected each day before use.
1926.451(f)(7): Scaffold not erected by competent person.
1926.451(f)(16): Platforms shall not deflect more than 1/60 of the span when loaded.
1926.451(g)(1): No fall protection provided.
Corrective Action:  Erect scaffold in accordance with the scaffold standards, 1926, Subpart L.

Hazard:  Fall from scaffold.
Standard violated:  1926.451(b), 1926.451(e): Employee climbing cross braces, platform not fully planked.
Corrective Action:   Provide safe access. Fully plank scaffold platform.

Hazard:  Fall from scaffold.
Standard violated: 1926.451(b)(1): Scaffold platform not fully planked.
1926.451(b)(4): Planking not extended over supports.
1926.451(c)(3): X-bracing not complete.
1926.451(g)(1): Inadequate guardrails provided.
1926.25: Housekeeping.
Corrective Action:  Provide competent person to oversee erection of fully planked scaffold platform with adequate overlap, cross bracing and guardrail system.  Insure adequate house keeping.

Hazard:  Fall from scaffold.
Standard violated: 1926.451(b)(1), 1926.451(b)(2), 1926.451(b)(4), 1926.451(g)(1):  (1) Platform consists of one plank only; (2) Plank is supported by another plank that is supported by a window sill; (3) No fall protection.
Corrective Action:  Provide adequate support for planks. Fully plank platform. Provide fall protection.
Hazard:  Scaffold not fully planked; Scaffold boards extending too far beyond supports; lack of fall protection; Absence of head protection.
Standard violated:
1926.451(b)(1) 1926.451(b)(5)(i) 1926.451(g)(1) 1926.100(a):  Apartment complex construction with the use of very poorly set-up scaffolding.
Corrective Action: 
Install proper planking and guardrails and provide and require the use of hardhats.

Hazard:  Fall from scaffold

Standard:  1926.501(b)(1) or (13), 1926.451(c)(1)Employees working on the top level of roof greater than 6 foot fall without adequate fall protection.  Cross braces are not installed at all locations creating a possible stability hazard on this tubular welded scaffold.

Corrective Action:  Employer needs to provide fall protection to employees at all working levels and the fall distance needs to be limited to 6 foot fall distance. Reference 1926.502 for Fall protection systems and criteria and practices.
Provide cross bracing at all locations where the horizontal members support the outer and inner legs. See manufacturer's guidelines.

Hazard:  Fall from scaffold

Standard:  1926.451(b)(1) and (g)(4)(i):  missing planking and guardrails on tubular welded frame scaffold.
Corrective Action:  Each platform on all working levels of scaffolds shall be fully planked or decked. Guardrail system shall be installed along all open sides and ends of the platform.

Hazard:  Fall from scaffold

Standard:  1926.451(b)(2), 1926.451(f)(3), 1926.451(g)(1), 1926.452(g)(1), 1926.452(g)(2):  Employees were placing siding at the top of the incorrect scaffold.
Corrective Action:  Properly erect the scaffold in accordance with OSHA standards.

Hazard:  Fall from scaffold.
Standard violated:  1926.451(c): Scaffold not plumb. Cross bracing missing.
Corrective Action:  Plumb scaffold. Install cross braces.
