Hazard:  Fall hazard

Related Standard:  1926.501(b)(1):  Employee climbed out of scissor lift without fall protection.

Corrective Action:  Require employee to utilize 100% fall protection when entering/exiting an elevated scissor lifts.

Hazard:  Fall hazard

Related Standard:  1926.501(b)(1):  Employees were taking measurements for the new metal roof.
Corrective Action:  Employees used an aerial lift to perform their job

Hazard:  Fall from unprotected sides and edges.
Standard violated:  1926.501(b)(1): Missing mid-rail on an open-sided floor of a building. This is possibly exposing employees to an 11.5 feet fall.
Corrective Action:  Fall protection must be provided for any employee on a walking or working surface with an unprotected side or edge which is six feet or more above a lower level. If guard rails are used for this protection, they must be equipped with a top rail approximately 42 inches (plus or minus 3 inches) above the walking or working surface, with midrails, screens, mesh, intermediate vertical members or equivalent structural members installed between the top edge of the guard rail and the walking or working surface. If midrails are used, they will be installed midway between the top edge of the guard rail and the walking or working surface.

Standard violated:  1926.501(b):  Employee on tower crane w/o fall protection. Fall hazard of 120 feet.
Corrective Action:  Require employee to utilize 100% fall protection when climbing tower crane.

Hazard:  Fall through hole.
Standard violated:  1926.501(b)(4): Employees were not protected by guardrail systems, covers, or personal fall arrest systems.
Corrective Action:  Install covers, adequate guardrail system, or personal fall arrest systems.

Hazard:  Fall from runway.
Standard violated:  1926.501(b)(6): Guardrail system not provided.
Corrective Action:  Provide guardrail system.

Hazard:  Fall from elevation.
Standard violated:  1926.501(b)(10), 1926.501(b)(11), 1926.501(b)(13), 1925.503(a)(1):  Employees performing roofing work on a residential construction (home) is exposed to fall hazard.  employee is carrying felt to complete the roof installation at a height over 6 feet.
Corrective Action:  Employer needs to evaluate the height to determine minimum fall requirements and necessary training.  Reference - STD 3.1. for falls less than 25 feet and require conventional fall protection for falls greater than 25 feet.

Corrective Action:  Provide fall protection (See Directive STD 3-0.1A).

Hazard:  Fall from elevation.  Falling objects.
Standard violated:  1926.501(b)(13): Fall protection not provided.

Corrective Action:  Provide fall protection (see Directive STD 3-0.1A).  Provide head protection.

Hazard:  Fall from floor joist (residential construction).
Standard violated:  1926.501(b)(13):

Corrective Action:  Provide fall protection (see Directive STD 3-0.1A).

Hazard:  Fall from unprotected edges.
Standard violated:  1926.501(b)(15): Fall protection not provided.
Corrective Action:  Provide guardrail system, safety net system, or personal fall arrest system.

Hazard:  Fall hazard.

Related Standard:  1926.502:  Employee exposed to the hazard of a fall while working on a billboard.

Corrective Action:  Employee should be provided with adequate fall protection system.
