setting up crane outriggers did not utilize fall protection. Ironworkers are
wearing full body harness with double lanyards. Both ends of lanyards are
attach to harness.
Corrective Action: Employees on a working/walking surface with
an unprotected side or edge more than 6 feet above the lower level shall be
protected from falling by use of guardrail systems, safety net systems, or
personal fall arrest systems. |

exposed to 35-foot fall hazard while standing on tie-beam without fall

Standard violated: 1926.501(b)(1); 1926.701(b),
1926.451(g)(1), 1926.451(e)(1): Employees are exposed to a fall hazard
greater than 6 feet, while working on the ledge of masonry block wall
without fall protection. Employees are working adjacent to unprotected
protruding reinforcing steel (rebar) which creates an impalement hazard.
Employees are sitting on the scaffold working level without any fall
protection, exposing themselves to a fall of over 10 feet.
Corrective Action: Provide a fall protection system or work
platform meeting the requirements of OSHA standards. Reference 1926.502.
Provide adequate covers over reinforcing steel. Provide standard
guardrail system consisting of a top rail, midrail and toeboard as
required. The employer needs to verify that the scaffold frame side rails
are manufactured for employees to access the various working heights. |
