Fall Hazards - Page 1

Hazard:  Walking/working surface collapse.
Standard violated:  1926.501(a)(2): Walking/working surface was not strong enough to support employees safely.
Corrective Action:  Ensure that walking/working surfaces are capable of adequate strength.

Hazard:  Fall from ladder and unprotected sides and edges.
Standard violated:  1926.501(b): No fall protection provided from unprotected edge.
1926.1053(b)(1): Ladder extends less than 3 feet above landing.
Corrective Action:  Use guardrail systems, safety net systems, or personal fall arrest systems.  Use longer ladder.

Hazard:  Fall from unprotected edge.
Standard violated:   1926.501(b)(1):  Ironworkers setting up crane outriggers did not utilize fall protection. Ironworkers are wearing full body harness with double lanyards. Both ends of lanyards are attach to harness.
Corrective Action:  Employees on a working/walking surface with an unprotected side or edge more than 6 feet above the lower level shall be protected from falling by use of guardrail systems, safety net systems, or personal fall arrest systems.

Hazard:  Fall hazard of 35 feet.
Standard violated:   1926.501(b)(1):  E
mployee exposed to 35-foot fall hazard while standing on tie-beam without fall protection.
Corrective Action: 
Provide guardarils, a personal fall arrest system, or a safety net for employee protection.

Hazard:  Fall from unprotected edge.
Standard violated:   1926.501(b)(1); 1926.701(b), 1926.451(g)(1), 1926.451(e)(1):  Employees are exposed to a fall hazard greater than 6 feet, while working on the ledge of masonry block wall without fall protection. Employees are working adjacent to unprotected  protruding reinforcing steel (rebar) which creates an impalement hazard.  Employees are sitting on the scaffold working level without any fall protection, exposing themselves to a fall of over 10 feet.
Corrective Action:  Provide a fall protection system or work platform meeting the requirements of OSHA standards.  Reference 1926.502. Provide adequate covers over reinforcing steel.  Provide standard guardrail system consisting of a top rail, midrail and toeboard as required.  The employer needs to verify that the scaffold frame side rails are manufactured for employees to access the various working heights.

Hazard:  Fall from unprotected edge.
Standard violated:   1926.501(b)(1) [Note: New construction assumed]
Employees not protected by guardrail systems, safety net systems, or personal fall arrest systems.
Corrective Action:  Install guardrail systems, safety net systems, or personal fall arrest systems.

Hazard:  Fall from roof.
Standard violated:  1926.501(b)(1) (Note: assume no roofing activity): Fall protection not provided.
Corrective Action:  Provide guardrail systems, safety net systems, or personal fall arrest systems.

Hazard:  Fall from pallet on forks of forklift.
Standard violated:  1926.501(b)(1):

Hazard:  Fall from bridge.
Standard violated:  1926.501(b)(1): Fall protection not provided for unprotected edge.
Corrective Action:  Provide guardrail system, safety net system or personal fall arrest system.

Hazard:  Fall from bridge.  Danger of drowning.
Standard violated:  1926.501(b)(1): Fall protection not provided for unprotected edge.
1926.106: Employees not protected against drowning.
Corrective Action:  Provide guardrail system, safety net system or personal fall arrest system. Provide life preservers, ring buoys and lifesaving skiff.

Hazard:  Fall from bridge.
Standard violated:  1926.501(b)(1), 1926.1053(b)(1): Fall protection not provided.  Portable ladder does not extend above landing.
Corrective Action:  Provide guardrail system, safety net system or personal fall arrest system.

Hazard:  120' Fall hazard. Employee was working at the edge of the roof and at times leaning over the edge without fall protection.  Horizontal lifeline erected, however employee was not wearing a body harness.
Standard violated:  1926.501(b)(1): Employees working near unprotected edges were not provided fall protection.
1926.503(c): Employees need to be retrained in fall protection requirements.
Corrective Action:  Provide guardrail system, safety net system or personal fall arrest system.
